Anxiety and self-doubt

Why Do I Understand My Anxiety but Still Feel Stuck?

Understanding where anxiety comes from does not always stop the reaction. Julie Cochrane explains the difference between insight and an automatic learned pattern.

The short answer

Because understanding is conscious, while an anxious reaction can be automatic. Knowing why you scan for danger, people-please or doubt yourself can be useful without changing the response the next time the trigger appears.

Insight is valuable, but it is not the same as change

You can know exactly where a pattern began and still feel your chest tighten before a difficult conversation. You can understand that your fear of disappointing people is old and still say yes before you have checked what you want.

That is not proof that you are failing at therapy or that you lack willpower. It may simply mean that the explanation has reached your thinking mind while the response is still being generated automatically. NIMH describes anxiety as something that can show up across situations and interfere with daily life; recognizing that pattern is a useful first step, not a demand to fix it instantly.[3]

Julie describes this as the difference between recognising a pattern and changing the pattern. Her RTT work focuses on the meaning attached to a recurring response, then gives you a way to relate to it differently. That is her approach, and it is not a guarantee or a replacement for other care.

What being stuck can look like

The pattern often appears in the gap between what you believe and what you do. You may know you are safe but keep preparing for disaster. You may know you are allowed to rest but feel guilty the moment you stop. You may know another person's disappointment is survivable but still organise your life around preventing it.

The useful question is not “Why don't I know better?” It is “What does this response believe it is protecting me from?” That question creates room for compassion and gives a discovery conversation somewhere specific to begin.

A grounded way to think about it

Name the loop

Write down the trigger, the automatic response and what you do next. Look for repetition rather than judging the individual moment.

Separate explanation from blame

A learned response can make sense in context without being useful now. Understanding its origin is not the same as excusing harm.

Look for the cost

Notice what the pattern asks you to give up: rest, honesty, closeness, visibility or trust in yourself.

Choose the right level of help

A discovery call can explore whether focused RTT work fits; ongoing or severe symptoms may also need a regulated mental-health professional.

Questions people often ask

Does feeling stuck mean I need a different therapist?

Not automatically. Progress can be uneven, and different approaches suit different people and problems. Talk openly with your current therapist about what feels unchanged before making a decision.

Can RTT help if I already understand my childhood experiences?

It may be worth discussing if the same automatic pattern is still affecting your life. Julie's approach is not about collecting more explanations; it is about exploring the emotional meaning attached to a pattern and whether that is a suitable focus for RTT.

How do I know whether my anxiety needs clinical care?

If anxiety persists, worsens or interferes with daily life, arrange an assessment with a qualified healthcare professional. This page cannot diagnose an anxiety disorder.
